•         Alpha-blockers and antihypertensives: additive blood-pressure lowering can cause dizziness or hypotension; suitability needs professional review.

•         Strong CYP3A4 inhibitors: medicines such as ketoconazole or ritonavir can increase tadalafil exposure.

•         CYP3A4 inducers: some medicines can reduce tadalafil exposure and change response.

•         Other PDE5 inhibitors or ED treatments: the label advises against combining them because safety and efficacy of combinations have not been established.

Alcohol, grapefruit, food, and herbs

The current label warns that substantial alcohol use with tadalafil can increase dizziness, headache, heart-rate changes, and low-blood-pressure risk. This is more specific than simply saying “avoid all alcohol”; the exact label and individual medical advice should control.

The FDA explains that grapefruit can affect intestinal CYP3A4 for some oral medicines, but not every medicine is affected in the same way. Does a weaker response mean tadalafil resistance?

Tadalafil does not create antibiotic-style resistance. Inconsistent response may reflect the underlying cause, insufficient sexual stimulation, interaction effects, alcohol, health changes, anxiety, or how the medicine is being used.
